Job Summary

Responsible to the Director of Clinical Services Division for coordinating the departmental efforts at achieving Patient Centered Medical Home; assisting clinical sites in maintaining the certification once the PCMH recognition is obtained; maintaining knowledge of regulations, policies, and procedures related to the PCMH in order to assure continued compliance; maintaining open communication with Medical Directors at all the clinic sites in order ensure continued growth within PCMH framework; developing relationships with community and regional peer practices/organizations in order to learn from and share ideas and insights; coordinating and supporting PCMH committee (including staff, providers, and patients) in the planning of meetings, dissemination of information and following up on action plans; managing, organizing, and monitoring data related to Quality Improvement, Patient Satisfaction, and other related efforts that may evolve within the clinics; and performing other related duties as assigned.

Knowledge, Skills and Abilities
  • Knowledge of software programs (Microsoft Office preferred).
  • Knowledge of the medical field (especially pertaining to ambulatory care facilities).
  • Knowledge of residency programs.
  • Ability to work with little supervision.
  • Ability to organize information in a logical manner.
  • Ability to present information and communicate ideas professionally.
  • Ability to meet deadlines.
  • Ability to comprehend and communicate established guidelines, policies, and procedures.
  • Ability to develop and implement, and evaluate programs, plan events and coordinate committee actions.
  • Ability to manage programs and staff concurrently who are in different locations.
  • Ability to maintain confidential files, sensitive information, and detailed records.
  • Ability to interpret data and identify patterns.
Required Qualifications

Bachelor’s degree required (preferably in a health-related field); two to three years of responsible administrative experience.
